Michael Finamore, RN, Care Transitions Program

My job is to assist with follow-up care for patients after they're discharged. Those who are homeless, marginally housed or need medical respite. Care for these people starts in the hospital with a social worker and then my team takes over for the additional follow-up. I feel like this is an extremely important program within the hospital. It's my feeling that we, as an organization, need to really meet our patients where they are physically and mentally. Using the AIDET format in the hospital allows for a connection when conducting follow-up care. These are people who really need us so it means a lot to me to be there for them… and to be positive.
